Thank you for your interest in Beth Israel Preschool. On this page you will find all of the forms that need to be downloaded and sent in to the Preschool office in order to complete your child's registration FOR THE 2012-2013 SCHOOL YEAR. If you are interested in the current school year- 2011-2012, please call 601-956-6216 and speak with Calandra Brown. We also will need a copy of your child’s certified Birth Certificate and Mississippi Immunization Form 121. All forms must be mailed (Please, NO FAXES) to the preschool office at this address: 5315 Old Canton Road, Jackson, MS 39206. If you have any questions, please call 601-956-6216.
